Chronic Disease Management (formerly Enhanced Primary Care or EPC) is a program aimed at patients suffering from chronic pain conditions and other various acute conditions. This Medicare rebate system is designed to assist with healthcare costs & refund part of the cost of attending a private allied health service. A CDM referral allows you to have up to 5 allied health services within the calendar year and is valid for a 12-month period.
